Chiropractor vs Doctor: When to See Which in Malaysia

When to see a chiropractor and when to see a doctor comes down to what is going on. A chiropractor is a sensible first stop for back, neck and joint pain that changes with how you move, sit or sleep. A doctor should come first when the pain comes with signs of something more, such as fever, numbness that is spreading, or pain after an accident, and whenever you need medicine or an MC for work. In Malaysia the two are different professions under different laws, and knowing that makes the choice easier.

Key takeaways

Is a chiropractor a doctor in Malaysia?
No. Chiropractors train in a chiropractic degree and register under the Traditional and Complementary Medicine Act 2016 (Act 775). They are not medical doctors, and cannot prescribe medicine or issue an MC.
When should I see a chiropractor first?
For back, neck or joint pain that changes with how you move, sit or sleep, such as stiffness after desk work or pain on particular movements, with no warning signs.
When should I see a doctor first?
After an accident or fall, with spreading numbness or weakness, fever or feeling unwell, a sudden severe headache, any bladder or bowel change, or when you need medicine or an MC.
What if I am not sure who to see?
A GP is the safest first door. A chiropractor who finds something outside their scope should refer you to a doctor, and the two often work best together.
Do I need a referral to see a chiropractor?
No. You can book directly. If your assessment suggests you need a doctor or a scan first, a good chiropractor will tell you and help you arrange it.

Is a chiropractor a doctor in Malaysia?

No. A chiropractor is not a medical doctor. Chiropractors here train in a dedicated chiropractic degree and register under the Traditional and Complementary Medicine Act 2016 (Act 775), where chiropractic is its own recognised practice area. Medical doctors, including GPs and orthopaedic specialists, register with the Malaysian Medical Council.

The difference matters in practice. A chiropractor assesses how your spine and joints move and works on them by hand, but cannot prescribe medicine, issue a medical certificate for sick leave, or perform surgery. A doctor can do all three, and is the right person to rule out illness behind the pain. Neither is a replacement for the other, which is why a good chiropractor will tell you when you need a doctor first.

At our clinic chiropractors and physiotherapists work side by side, so if your problem is better suited to rehab than adjustment, you do not have to start again somewhere else.

When to see a chiropractor first

A chiropractor is a reasonable first stop when the pain is mechanical, meaning it comes from how your body is moving and being loaded. Common signs:

  • back or neck pain that is worse after long sitting and better when you move, or the other way round
  • stiffness first thing in the morning that eases as the day goes on
  • pain on particular movements, such as turning your head to reverse the car or bending to tie your shoes
  • an ache that builds over a week at the desk and settles at the weekend
  • pain that keeps coming back in the same place every few months

UK guidance for low back pain recommends exercise, and suggests considering manual therapy such as spinal manipulation only alongside it. Assessment should still come first. At our clinic in Cheras that means a history, a physical and neurological check and, where needed, an X-ray before anything is adjusted.

When to see a doctor first

See a doctor, or go to the emergency department where it says so, before seeing a chiropractor if you have:

  • pain after a fall, a sports injury where you cannot bear weight, or a road accident
  • numbness, pins and needles or weakness in an arm or leg that is spreading or getting worse
  • loss of bladder or bowel control, or numbness around the groin (emergency department, today)
  • back or neck pain with fever, feeling generally unwell, or weight loss you cannot explain
  • a sudden, severe headache unlike any you have had before (emergency department)
  • chest pain or breathlessness alongside upper back pain (emergency department)
  • a history of cancer, osteoporosis or long term steroid use alongside new pain

A doctor is also the right first stop if you need pain medication, or if you need an MC for work. Under the Employment Act 1955, paid sick leave is tied to certification by a registered medical practitioner, which a chiropractor is not.

Who to see first, symptom by symptom

A quick reference for the most common situations:

  • Stiff, sore neck after a long week at the laptop: chiropractor. Our post on tech neck covers what helps at home.
  • Lower back ache that comes and goes with sitting: chiropractor. See our lower back pain exercises too.
  • Pain running down one leg, with no weakness or numbness: either. Our sciatica page explains what an assessment looks for.
  • Leg or arm getting weaker, or numbness spreading: doctor first, soon.
  • Pain after a car accident: doctor first, then a chiropractor once serious injury is ruled out. Our whiplash page covers the neck side of this.
  • Back pain with fever or feeling unwell: doctor first.
  • Needing medicine or an MC: doctor.
  • Not sure: a GP is the safest first door, and a chiropractor who finds something outside their scope should refer you on.

How chiropractors and doctors work together

Seeing one does not rule out the other, and the two often work best in sequence. A GP may rule out illness and suggest movement based care. A chiropractor may find signs that need a medical opinion or a scan and refer you back.

To make that easy, bring any scan reports or doctor's letters to your first chiropractic visit, and tell your doctor you are seeing a chiropractor. If an X-ray is needed during your assessment with us, we send you to the radiology department at Sunway Medical Centre Velocity, a short walk from our door, and go through the results with you.

Frequently asked questions

Can a chiropractor give me an MC for work?

No. Under the Employment Act 1955, paid sick leave is tied to certification by a registered medical practitioner, and chiropractors are not medical practitioners. If you need an MC, see a doctor.

Can a chiropractor order an X-ray?

Where an X-ray will change the plan, we send you for one at the radiology department of Sunway Medical Centre Velocity, a short walk from the clinic, and go through the results with you. It is not needed for everyone.

Can I see a chiropractor and a doctor at the same time?

Yes, and many people do. Tell each of them about the other, and bring any scan reports or letters with you, so neither is working with half the picture.

Is it safe to see a chiropractor while I am on medication?

Usually, but tell your chiropractor what you take. Blood thinning medication, long term steroids and some other conditions change what is appropriate, and the assessment takes them into account.

Should I see a chiropractor or a physiotherapist?

Both are reasonable for mechanical back and neck pain. Chiropractic focuses on how joints move, physiotherapy on exercise and rehabilitation, and at our clinic the two work together, so the assessment decides where to start.

Chiropractor or doctor: the bottom line

For pain that changes with how you move, sit or sleep, a chiropractor is a sensible first stop. For pain that comes with fever, spreading numbness or weakness, a recent accident, or a need for medicine or an MC, start with a doctor. If you are unsure, start with whichever you can see sooner and expect each to refer you to the other when it is needed. If you would like an assessment, our page on Gonstead chiropractic care in Cheras explains what the first visit involves.
