What causes most back pain?
Most is mechanical, meaning it comes from how the spine is being loaded and moved rather than from serious disease. That is why the assessment looks at movement before it looks for anything else.

What contributes to it

Why it happens

Prolonged sitting
Long hours at a desk load the lower back continuously and let the muscles that support it switch off. This is the single most common contributor we see in Kuala Lumpur, and it responds well to a combination of hands-on care and changes to how you sit.
Lifting and sudden loading
Lifting something heavy, awkwardly, or while twisting can strain the joints and soft tissue of the lower back. The pain often arrives hours later rather than at the moment of lifting.
Joint restriction
When one spinal segment stops moving properly, the segments above and below compensate by moving more. Over time those neighbouring joints become irritated. Finding the restricted segment is what the Gonstead assessment is for.
Deconditioning
Back pain that keeps returning usually has little to do with one injury. More often the supporting muscles lack the endurance to hold the spine in a decent position through a full working day, so the same tissue gets irritated again every few weeks.

Our approach

  1. We find the segment before we work on it

    Your first visit follows the Gonstead six-step assessment: history, visualisation, instrumentation, palpation, X-ray analysis where indicated, and only then an adjustment. We want to know which specific joint is producing your pain before anything is adjusted. Adjusting the whole spine and hoping is not how this works.

  2. Adjustment, delivered by hand

    Once the segment is identified, the adjustment is specific to it. Your chiropractor will explain what they found and what they intend to do before anything happens.

  3. Then the part that stops it returning

    An adjustment restores movement to a joint, but the muscle around it still has to be able to hold that position through a long day. Where the pattern is a recurring one, we pair chiropractic with physiotherapy-led rehabilitation and practical changes to your desk setup and daily loading.

How long does back pain take to settle?

It depends on how long you have had it, what is causing it, and what you do between visits. Short-term relief often comes quickly. Changing how your spine moves and rebuilding the muscle that supports it takes longer, usually months rather than weeks. Your chiropractor will give you a timeline specific to your case after the first assessment rather than a generic number.

Do I need an X-ray for back pain?

An X-ray gives us a clearer picture of what is happening in your spine: how each segment is positioned, how the discs and joints look, and whether anything is present that should change the plan. That is what makes the Gonstead assessment specific to your back rather than general. It is not mandatory for every patient, though, and it is avoided for pregnant women and children unless there is a clear reason. Your chiropractor will explain why an X-ray is or is not indicated in your case.

Should I rest or keep moving?

For most mechanical back pain, gentle movement helps more than bed rest. Light walking stimulates blood flow and stops the area stiffening further. Avoid movements that clearly aggravate it, and hold off on vigorous exercise for a few days after an adjustment.

Is chiropractic safe if my back pain is severe?

Severity alone does not rule out chiropractic care, but it does change the assessment. Some presentations need imaging or a medical referral first, and if your chiropractor believes another healthcare provider would serve you better, you will be referred. Tell us about any recent trauma, numbness, or weakness when you book.
