Chiropractic vs Physiotherapy: How to Actually Decide

· Persistence Chiropractic Care6 min read
Treatment room at Persistence Chiropractic Care in Cheras, Kuala Lumpur

Chiropractic and physiotherapy get compared constantly, and most comparisons stop at the same place: one works on joints, the other works on muscles, book whichever sounds closer to your problem. That is not wrong, but it skips two things worth knowing. In Malaysia the two sit under entirely different regulation, and which one actually fits depends more on the specific condition than on a general rule. We have a full side by side table of what each involves on our physiotherapy page. This is the part that table does not cover.

Key takeaways

Are chiropractic and physiotherapy regulated the same way in Malaysia?
No. Chiropractic is one of the recognised practice areas under the Traditional and Complementary Medicine Act 2016 (Act 775), registered through the T&CM Council. Physiotherapy falls under the Allied Health Professions Act 2016 (Act 774), registered with the Malaysian Allied Health Professions Council.
What does each mainly work on?
Chiropractic care works on how a restricted spinal joint moves, most often through a hands on adjustment. Physiotherapy works on the strength and control around a problem, through movement assessment, hands on care and a structured exercise programme.
Does the same condition always call for the same one?
No, it genuinely varies. Sciatica and neck pain often lean toward physiotherapy and dry needling first, while back pain, a slipped disc, scoliosis and migraine can draw on both, and which starts first depends on what an assessment finds.
Is it normal to need both?
Yes. A joint that has been stuck for a while often carries tight muscle around it, and freeing the joint does not automatically retrain that muscle, so combining both is common rather than a sign either approach fell short.
When is neither the right call?
When the problem is not mechanical: infection, fracture, or disease affecting an organ. A practitioner who takes that seriously refers you to a doctor rather than adjusting or exercising you regardless.

Chiropractic vs physiotherapy: two different Acts in Malaysia

Chiropractic and physiotherapy are both regulated healthcare professions in Malaysia, but under separate legislation entirely. Chiropractic is one of the recognised practice areas under the Traditional and Complementary Medicine Act 2016 (Act 775), registered through the T&CM Council alongside traditional Malay medicine, traditional Chinese medicine, traditional Indian medicine, homeopathy, osteopathy and Islamic medical practice. Physiotherapy falls under the Allied Health Professions Act 2016 (Act 774), and physiotherapists register separately with the Malaysian Allied Health Professions Council.

Neither framework makes one profession more legitimate than the other. What it means practically is that a chiropractor's registration does not qualify them to practise physiotherapy, and a physiotherapist's registration does not qualify them to adjust a spine the way a chiropractor does. If you are checking credentials, ask which registration a practitioner actually holds rather than assuming either title covers both.

What chiropractic care and physiotherapy each work on

Broadly, chiropractic care works on how a restricted spinal joint moves, most often through a hands on adjustment aimed at a specific segment. Physiotherapy works on the strength and control around a problem, usually through movement assessment, hands on care and a structured exercise programme you continue between visits. Neither is better in the abstract, and the full breakdown of first visits, main tools and what each suits sits on our physiotherapy page rather than repeated here.

Chiropractic or physiotherapy: which fits which condition

This is where a general rule stops being that useful, because it genuinely depends on the condition. Looking at how we have built out our own condition pages: sciatica and neck pain lean toward physiotherapy and dry needling first, since movement and muscle tension tend to drive both. Back pain, a slipped disc, scoliosis and migraine draw on both chiropractic care and physiotherapy, and which one starts first depends on what the assessment finds. Hip pain leans more toward chiropractic care alongside sports injury rehabilitation.

None of that is a fixed rule, and it should not be read as one. It reflects what tends to help most people with that presentation, not a promise of what will happen in any individual case, which is exactly why an assessment comes before a plan rather than after one.

Can you see a chiropractor and a physiotherapist at the same time

Plenty of people benefit from both, and it is a genuinely common combination rather than a sign that one approach failed. A joint that has been stuck for a while often carries tight, guarded muscle around it, and freeing the joint does not automatically retrain the muscle to behave differently afterwards. Under one roof in Cheras we offer chiropractic care, physiotherapy and dry needling together, and the assessment decides where to start and whether both make sense for your case, rather than which one you happened to book first.

What a first chiropractic or physiotherapy visit looks like

Both start with an assessment rather than jumping straight to hands on work, though the shape differs. A chiropractic first visit generally follows a structured, segment by segment analysis before any adjustment. A physiotherapy first visit centres on how you move: gait, posture and load, followed by hands on care and your first exercises to take home. Neither should skip straight past this stage, and if a practitioner adjusts or starts hands on work within the first couple of minutes regardless of what you came in for, that is worth being cautious about.

When neither chiropractic nor physiotherapy is the right call

Both chiropractic care and physiotherapy are aimed at mechanical problems: how a joint moves, or how a muscle behaves under load. Neither is built for infection, fracture, or disease affecting an organ, and a practitioner who takes that seriously refers you to a doctor rather than pressing on regardless. If your pain came with fever, unexplained weight loss, or started after a significant injury, that is a case for medical assessment first, not a chiropractor or physiotherapist.

Frequently asked questions

Which is better, chiropractic or physiotherapy?

Neither is better in the abstract. They work on different things and suit different presentations, so which one fits depends on what an assessment finds is driving your specific case.

Do I need a referral to see a chiropractor or a physiotherapist in Malaysia?

No, both can be seen directly without a referral. A practitioner who takes a proper history should still refer you onward if your case needs imaging or a medical opinion first.

Is it normal to be recommended both chiropractic and physiotherapy?

Yes, and it is a common combination rather than a sign that one approach did not work. Many problems involve both a joint that has stopped moving well and muscle that has tightened up around it.

Can the same practitioner be trained in both?

Registration in one does not cover the other, since they sit under separate Acts and separate councils in Malaysia. A practitioner would need distinct training and registration to legitimately practise both.

How do I know which one to book first?

If you are not sure, message a clinic your main concern before booking. A place that offers both under one roof can point you to a sensible starting point rather than leave you to guess from a website.

The bottom line

Chiropractic and physiotherapy are regulated differently in Malaysia, work on different things, and which one fits depends more on the specific condition than on a general rule. A fair number of people end up needing both, and that is normal rather than a sign either approach came up short. If you are not sure where to start, message us your main concern and we will point you to a starting point rather than book you into whichever one you happened to click.
